Can you actually make money on OnlyFans with no followers?
Yes. Everybody who has followers now started with fewer of them at some point.
That does not mean an empty account turns into income by itself.

The first problem is traffic. If nobody already knows who you are, you need some route that brings people to the page. Maybe separate social accounts. Maybe creator communities. Maybe collaborations, search, promotion or something else entirely. Whatever the route, the audience does not appear because you uploaded your first set.
That is why the "easy money" idea around OnlyFans can be misleading. Taking photos may be easy. Running an account that people continue paying for is a different job.
You may spend time editing, messaging, pricing, promoting, scheduling and planning what to make next. You also need to decide how much of your personality becomes part of the product and how much private life you keep completely separate.
Some people are very good at that and enjoy it. The business becomes part of the creative game. They test what gets attention, learn what the audience responds to and gradually build a recognisable persona.
If that sounds awful, notice it early.
There is another way to be paid for adult content without first building followers. A production can cast you for the project itself. The producer is taking the audience risk. You are being paid because you fit the role, look, chemistry or concept, not because you arrive with subscribers.
That does not mean produced work is automatically better money. It means the problem is different. You trade some control over distribution for not having to build the shop before somebody pays you.
A few strong productions can also become the beginning of a stage identity if you decide to continue. Then, if you later launch your own creator account, you may be starting with a name and material rather than from complete zero.
So the real question is not simply whether you can make money with no followers. It is whether you want your first job in the business to be getting followers.
There is also a timing problem. Building from zero rarely happens in one clean burst. You can make excellent content and still spend weeks learning what attracts the wrong audience, which posts get ignored and what kind of promotion you simply hate doing. The first few months can be more like learning a small business than cashing in on one.
That is not an argument against starting. It is just worth comparing the workload honestly with other routes. If what excites you is actually the photography, sex, acting or being on camera, then spending most of your energy learning audience acquisition may be a strange way to enter the industry.
